—Our Miles of Smiles (MOS) mobile dental unit is scheduled to be in Holland throughout the school year. Our mobile dental unit provides free, preventive and restorative dental services for financially-qualifying children. Unfortunately, MOS will not be visiting your school this year. However, we want parents to know that they can schedule their child to be seen on our mobile unit when we locate at a school in the Holland School District or West Ottawa School District. The forms can be accessed on our website: http://www.miottawa.org/dental. If any consent forms are returned to your office, please fax to (616) 393-5643.
–If your child will not be in school you MUST call the attendance line at 748-3205. Reminder – if your child will be out of school, you may email their teachers, but you also need to call the attendance line or office. Thanks!
All signups will be done online in the Final Forms www.zeeland-mi.finalforms.com
. Parents and students need to register once a year for all your sports you will be playing or think you might want to play. Please check your mail/spam mail for confirmation once you have originally signed into Final Forms to confirm your account. You can also link Final Forms through the cityside.zps.org
website under athletics, or you may pick up a hard copy of the Final Forms instructions by the CBC room or in the office at Cityside. Like in the past we need a current 2018-2019 physical dated from the doctor after April 15
